Answer:
you will need to file a petition for name change. you can obtain the form online. for example, if you are in california, you can go to courtinfo.ca.gov to obtain a form. there should be similar ones in other states. once you file it, you'll have to wait for a decree or order from a judge making the name change official. you can then take the decree/order to the dmv, social security, immigration, and get your name changed.
