I have a 5 years F1 visa (D/S) issued to me based on an I-20 by an undergrad school. I have finished my undergrad studies and now i am moving to a Grad. school, so i will be getting a new I-20 from a school other than the one on my visa, which is going to expire very soon, so i will be staying on duration status. But can i visit home while iam attending Grad school, and then be able to re-enter to the US just on my new I-20 and an expired visa but still on Duration status? Or will i be required to stay home and apply for a new visa and have to wait for it, thus jeopardizing my school year?

No, you must have a valid visa in order to return to the US. "Duration of status" is only for those who are staying in the US while they study. As soon as you leave, you must use your new I-20 to apply for a new visa.
Why do you think you'll jeopardize your school year? You can make your interview appointment even before you leave the US, using the information available on the web page of the embassy or consulate where you will apply.
You'll need to apply for a travel document that describes
your situation. If you do not have a travel document, don't
do it.
