Unfortunately you are correct that the Home Office often lose documents. This even aplies if you have hand-delivered it. I think that you should seek legal advice to clarify your situation; if appropriate your solicitor would send your passport by special delivery, that way there would at least be a record of it being sent. Of course you can send it yourself by special delivery (costs about £3). The Home Office has two main offices, at Lunar House in Croydon and in Liverpool. You really really don't want to go to Lunar House.
If you have been continually resident in the UK for 14 years you can apply for Indefinite Leave to Remain, but I think a visit abroad would break your continuity of residence. The fact that they let you back in, even if you explained that you live here rather than were just visiting, suggests that your situation should be OK. But get legal advice before parting with your passport.
