Indian Passport for minor and applying for a duplicate passport in case of damage?

I want to apply for passport of my 6 months old daughter.

Me and my wife are holding a valid passport that is issued from Jaipur. We are staying in

Hyderabad from past 11 months. Before that we were in Bangalore for 6 months.
We do not have any address proof of Bangalore though.

We have our marriage certificate and our daughters birth certificate. I can also get bank statements of past 11 months containing my Hyderabad address.

Unfortunatly my passport is damaged.

I have following queries:

1. Can I apply for a duplicate passport for myself at Hyderabad, if yes what are the documents needed?
2. In the same form can I apply to include my wife's name in my passport?
3. Can I apply for my daughters passport now or I have to wait until I get duplicate passport?
4. What are the doucments required for my daughters passport?
5. What are the additional documents needed incase I want to apply for the above two in tatkaal scheme?
