First of all, an American can legally visit Canada for up to 6 months without a visa. Adiditonally, that visit can be extended when the 6th month approaches. So the American could initially go to Canada to visit, stating that they are visiting a friend. If the person states that they are visiting a fiance, Canadian Immigration will question whether you are planning on staying in Canada, in which case they may deny the American entry. However, stating that the friend is disabled and that you are going to help him/her will give reason to your lengthy stay. Immigration will definitely ask you how you expect to afford to stay in Canada so long. In which case you may need to show proof of funds. They will alos ask you if you are employed.
The Canadian fiance could possibly sposor the American as long as they are not on public assistance. I believe that disability is not considered the same thing for sponsorship purposes, but you may want to check on that.
Otherwise, if they will not or can not sponsor the American, the American can come to Canada on a work Visa but will need a job offer before getting the visa (this is very hard to do). They can come to Canada with a student visa but will need to show the colleges letter of acceptance. They can come under the skilled worker program.
Without really know the American's situation/education/employment status, it hard to say.

The American if owns his home should rent it out, where he'll have steady income or sell it, where he'll have more than enough of the $10,000 fee needed. Get any severance pay from his job (if possible). Make his way to you, and have a court judge marry the two of you at a small cost.
