If you came to the U.S. with the intention of getting married, even if it was a slight possiblity, yes, you need to go back to Italy and have your wife file the I-130 and as soon as it is approved file for the K-3. You can be deported for fraud and misrepresentation at the time of entry if there was even a possibility you were going to get married. That is, unless you can demonstrate to the powers that be that it was your intent to return to Italy immediately after your nuptuals to do things the right way.
Now, if you were already here legally (student, exchange visitor, work visa, etc) and this is just a natural occurence of life - hie thy butt down to the USCIS office and submit all the paperwork for adjustment of status. As long as you were legally here already and fell in love and got married you are fine unless you 1) don't file for adjustment of status in a timely manner, or 2) leave the U.S. after filing for adjustment of status without first obtaining an advance parole (Form I-131).

bring your wife back to italy and apply for papers there but before you do ask the ins for some correct info first!!! i just want to give your wife some nice place to go to and italy is one gorgeous place to visit.

You are coming across as getting married only to get residency. Wish you had gone on further with your question to say, "I need this, as I can't bear to be away from my wife, should everything not go alright."
See if Italy offers a DCF option - Direct Consulate Filing. It allows your wife, after living in your home country for X months, to file directly for your greencard at the US consulate in your country. They have it in India and that is how I brought my husband over with a greencard in less than 8 months after we got married. Check out the links below:
If you still have time on your visa, apply for the K here and when it is approved, then go back after you already have an appointment at the consulate. That way you will only be away from her a few weeks.
