Yes, you need one. I-94 is a form that is also known as arrival/departure record. It is a form that is handed to any person who is legally entering the USA. If you are travelling by plane, then the plane steward/stewardess will hand it to you prior to landing.
You will fill it out and submit it at the port of entry. It will be stamped with the date of entry and the last day that you are allowed to stay here. It will be stapled to your passport, and on the day of your depature, it should be surrendered (if leaving by plane, it should be surrendered at the airline counter).

It will be given to you upon entry, fixed in your passport, and the Immigration Officer will enter the date by which you must depart on it. When you leave it will be collect at the port of exit.

If your visa is only 1 time entrance, you will not be able to come visit us, then canada, then us again. Maybe you can go to canada first, then come to us?
