Could I bring my new DS-2019 to apply the new USA visa in the US?
If it's ok, which institution will I go?

Wait, what visa are you currently on? If you're already on a J-1 visa, you should be able to just extend and get a new DS-2019 without leaving the country.
If you're on a different visa type, you may apply for a Change of Status (COS) with the form I-539. However, wait times for this can take up to six months and that'll put you past your expiration date of your current visa.
If you're applying for a J-1 and you're outside the country, you will have to do a visa interview at home. Once you have your J-1 visa stamp, you can come into the country up to 30 days before the program start date.
DS-2019s are for specific schools. When you go to the visa interview, you'll need to take the DS-2019 with the information for that specific instituation (ie. acceptance letter, employment letter, sponsor letter, etc.). you'll also get a stamp on your DS-2019.

Get in contact with the USCIS NOW!!!!!, because the expiration date is very soon and you need that time to apply for the need visa. If you do so it may just have you stay here and have it extended.
The more time you have in between applying for your new visa and the expiration date, the better.
