Where do you take the US citizenship exam? and what must be done before taking the exam?

In other words how do you sign up to take the exam, or where?

Answer:
You don't sign up to take a exam.

You need to file a form called N-400 with all the requirements (documents, money) that go with it. Depending on what state you live in, you send it to one of the Service Centers the USCIS has all over the country.

The form can be filled on the Internet and then printed.

After that, they will contact you and tell you what to do next and where. You only follow orders.
