I am filling up the ds-156 forms for my children both are below 12 years old, do I have to sign item 41?

Item 41 on the DS 156 form is where the applicant's signature will be placed. He/she certifies that he/she have read and understood all the questions set forth in the application. But the forms were prepared by me on their behalf. I think I will be the one who will signed it, I am right?

You are right. But you don't sign line 41; you check box YES on line 39, then complete and sign the applications on line 40 (not 41) for your children.
