You'll need both parents to apply in person. If one parent can't be there then a notarized affidavit from the absent parent must be submitted.
It doesn't matter if the parents are divorced. You still need both of them. A court order showing sole custody is usually NOT enough since most such orders restrict the custodial parent's right to take the child out of the country. If you can't get the other parent to agree you'll need a court order signed by a judge.
If one parent is dead you need the death certificate.
If the father is unknown you need a birth certificate listing only one parent by name.
Don't have kids, but from what I saw when I recently renewed mine, it's pretty much the same deal - parent or legal guardian has to be there, of course, but it's not much different than an adult getting a passport.

You go the same place - to the State Department web page or the US Post Office for forms (this is most post offices where I live, but some don't do it. You can check on-line). If you have a court order showing sole custody you can use that to apply on your own for the child. Otherwise it needs the signature of both parents, to prevent child abduction.
Go to travel.state.gov. You will find any information you need there. To get the passport for a minor child of divorced parents; you will need the signature of both parents.
Both parents have to sign for a passport. Sorry it doesn't matter if they are divorced.
