How do i get a form ! -131 ?

i need to obtain , fill out , and have done whatever needs to be done to get a re entry form so i can leave the staes and then come back after vacation ( cruise) is done, I have had a valid "green" card for more than 40 years , recently renewed.

If you have a valid green card, you do not need a re-entry permit unless you will be away for more than one year. That is a very long cruise!
