You are supposed to undergo a full physical/medical/psychological exam for a visa application. And to apply for a greencard, unless you've recently had a medical exam (within the last 12 months) you do need to undergo yet again another medical exam.

You have to take a blood test and they do give you and AIDS test. Maybe the rules changed in the last few years, but I doubt that they test for specific drugs.
yes, a full medical exam is required and has to be done by an approved civil surgeon. vaccinations, tb tests, HIV tests, etc. are required. check out form I-693 and the supplemental form on the www.uscis.gov site under forms. the dr. checks for signs of drug/alcohol abuse.
