Depends on what you want to do.. Otherwise just get a visitors visa like any other person, they are good for 3 months I believe.
Just a passport I think, not 100% though. Try asking someone at the american consilate
You can get a visa waiver with your passport at the border to enter the US
Your passport is enough to get back to Canada (and be a nice person)
The USA has a visa waiver scheme with UK Citizens (you can LEGALLY stay in the US for 90 days) . Not sure if visa are needed for Canada, I don't think so.
