Contact social security office and they will let you know what you need to get a replacement. You can go on there website. http://www.ssa.gov. You should be able to find a link on how to obtain a lost or stolen ssn. Hope this helps.
You must go to your local Social Security office with your birth certificate and apply for a new card. Your number will be the same, it's just a replacement card.
If either of your parents is available, one or both can make a statement under oath of who you are. You can also hire a certified translator to translate your birth certificate.
Since you had a Soc. Sec card, they should have records on when you applied for it. They won't do the research work if they can get you to do it instead. Do you have pay stubs listing your name and soc. sec. number?
Sounds like a huge circle they are gonna put you in. You can by mail get your birth certificate and your parents birth & military records. I would send along a letter with the documents to the s.s. office. let them sort it all out.
In the state of Illinois all you need is some form of I.D. like a driver's license, I.D card,Medical card, or work I.D. and you will need to fill out a FF5 form if you want you can go to www.socialsecurity.gov on the page that comes up there will be form and publications click on forms then click on FF5 fill it out and have your I.D. with you when you go to your local social security office.
