i have a tourist visa. My wife is very ill and i am retired and cant afford health insurance. That is my biggest frear.

contact their nearest embassey/consulate offices---and,plead your case!
I wouldn't overstay my visa if I were you. You should at least go to Mexico, then come back to the US, just to fullful the 'leave the US' requirement. You should try to stay in a legal status, then find a good lawyer to argue for undue hardship if you return to Venezuela due to your wife's status.
