Question about OCI - Chicago Office?

I am an Indian national and just recently got my US citizenship/passport.
The Chicago Consulate office doesnt have the best of websites, so I am looking to confirm a couple of questions regarding OCI applications:
1. Initially, I fill out forms A and B and send in money and copies of Indian and US passport. No originals. Is that right?
2. After approval and an application number, send in original US and Indian passport and they will send back canceled Indian passport and US passport with OCI visa. Is that right?
3. What is a self attested copy? They have asked for self attested copies of my Indian and US passport.
