Living in the USA...?

Does anyone know how to apply for a USA greencard/visa for an Australian citizen. Any website links would be appreciated. Also, if you travel to the US on a visitors visa is it possible to extend this to a visa to be able to live and work in the US for a long period of time, or do you have to apply for this type of thing before you leave your home country.
