Which country? England is fairly easy, except you have to ensure the baby has a totally white backgroung - ie, put a sheet over their carry cot/travel ch air - easier said than done,
In the US, I believe minor children are placed on the parent's passport.
As far as I know if the baby is below the age of 11-12 yrs, their names are included in the main visa holders passport.
The same as you would for anyone else. Ask the passport agency.
