Call the embassy or consulate for the country you´re from to obtain a new passport. Then take your drivers license (if you still have one, or wait for your passport) to SS admin and apply for a new card. Wait until you get your passport (how long depends on your embassy) and then make an appointment with the homeland security office closest to you to set up an appointment for a new green card. Go to the website below and it will give you tons of info. E-filing for a replacement card may be faster than going in. You should probably call the National Customer Service Center for homeland security at 1-800-375-5283 and report your card lost.
Firs call credit bureau agencies to warn them in case some one asks for credit in your name.

Also call your State agency that issues Driver's License let them know that there maybe an issue of identity theft.
