How do I get my "long-form" birth certificate?

I'm trying to apply for a passport, but I just found out that the Los Angeles passport agency no longer accepts the abstract form birth certificates, and my parents don't know where it is (they don't remember getting one)...How do I get the long-form?

Answer:
Call or mail the County Court House in county where U were born. They will tell U how. Been there-done that!
At the courthouse in the county you were born.
I think somebody was pulling your leg. A birth certificate needs to say your name, date and time of birth, the place you were born, the physician's name and signature, and your parents names. It's not that long of a form.
Hi there I had the same problem when I was getting my passport as well. The department had the forms I needed ready but you can also find them at the California office of vital records. http://www.dhs.ca.gov/chs/ovr/birthorder...

The actual form you'll need to fill out is here, it's the same form I filled out as well. Hope this helps. Good luck! Also make sure to check you want a certified copy.

Yes you have to have the long form, I went through the same hassle myself. Call vital records in the county you were born and they will tell you where to find the form to send in to get your long form cert. I think it costs around $15
