you can "port" your H-1B to a new position provided the following conditions are met:

1. the job is almost identical to your current job;
2. the wage offered in your new job meet state wage requirements; and
3. you are working in your current job on the day that you file your portability application.

It can be done and it is very common. You just have to find a new employer and a similar job first. The paperwork is almost identical to that of your original H-1B and the new H-1B will be valid for the duration of the orignal approval. You can extend the new H-1B as needed.

You have to find another employer to sponsor you and grant you another H1B. It can be done, but it's very difficult. You are supposed to leave the country when the original sponsor doesn't employ you any longer, but the rules are very vague about how long you have before you leave. The INS won't come knocking on your door the day after your job ends. Good luck!
I believe your new prospective employer will have to agree to sponsor you in place of your current employer....and you can have only one sponsor at a time. It is becomming decreasingly popular for employers to sponsor Visa applicants, as it is expensive. You need to be someone reeeeaaally specialized in your field. Stick with who you got. You have a visa right now.

The process called H1B transfer. Its easier then original since the cap doesnt apply to you.
