Hi Twin -- First we'll cover the generalities, then the specifics of Manila. You'll need to submit a DS-11 passport application, as well as 2 U.S.-sized passport photos (5 cm by 5 cm; at the photo shop be sure to specify this as it may be that Filipino passport photos are smaller). You will also need to provide the old passport, and the original birth certificate and/or Consular Report of Birth Abroad. The reason for this is a law that requires that both parents agree to the passport issuance (and they need to know who both parents are -- this is desgined to help prevent international child abduction). And... both you and the other parent on the B/C or CRBA need to be there to sign, as well as your son. If one parent is absent, he/she can provide notarized consent in English to the issuance of the passport. The cost is $82. There is the possibility that if your son's appearance has changed a lot since the last passport photo was taken, you may need to provide additional proof of identity like sequential growth photos or school records.
