Does Anyone know if I need to apply for I-131 form (Travel Documents, Reentry Permit) if I'm traveling outside U.S. as a permanent resident (not citizen) for less than a month.

All you need as an immigrant is to have your "green card" with you at all times, your valid passport (from your country of origin) and visas (from each country you are visiting if required). Otherwise, to re-enter the U.S.A., you will need your immigrant card with you.
