just relax do not panic
you just have to do only one single thing just go to the office of the district registrar, Birth and Death ,Municipal office and fill a form there in which you would be asked you particullars and then your form would be deposited and they would be giving you a date to collect as many copies of your birth certificate as requested by you

if its on you or you're child call the bureou of vital statistics and they will send or fax you one,,,,,,,,,they run the whole show i stopped an addoption by doing this on an unplanned daughter,,,,so dont worry they cant do anything..
