Answer:
Once you receive the fiance visa, you must marry within 90 days of arriving in the US. If you change your mind, the visa expires and you go back to the UK still single.
If you need a medical exam for the visa, you must have it with one of the embassy's panel physicians. They will tell you how to make the appointment.
Once you are married and have requested adjustment of status to get your green card, you shouldn't leave the US until the card is actually in your hands. If you leave, CIS considers that you have abandoned your adjustment request. Or you can request advance parole, which is permission to travel and return to the US while your green card is in process.
