Canada's a fairly hard place to get citizenship to. You have to prove that no one else can do your job (no other Canadians) if you're of age and it seems that because of that ruling a lot of immigrants don't live in the big cities.
I would say that since your brother is mentally disabled he will be rejected because he will be considered a strain on the government (hospitals, home care etc). If your parents are applying the same time as him (and sending the paperwork off in one big bundle) they all will be rejected.
Also since you're brother is 23 I would estimate that your parents are about 50.. that's close to retiring age although they may have another 10-20 years in them before they actually do so.
first of all it is not a family sponsored "scheme"... it is family sponsorship. It is true when acyla says that the government would deem your brother a financial burden on the health system due to his disability, that is part of the medical. A doctor says whether or not you would fit in one of the three or five categories.
the best you can do is call the call centre and explain everything to them and see what direction they send you. Perhaps you will have to go to a local immigration office to talk to an officer. if a lawyer is already telling you that they will probably be denied, you might have to look at another route.
