Answer:
A green cardholder still needs to re-apply for a visa when going out and coming in to the US. The greencard is only to prove being a resident of the US but not a proof of US citizenship.
The immigrant visa that was put into your passport was to use for your initial entry as an immigrant and then to travel - if you needed to - until your green card arrived.
If you now have a valid passport and a valid green card, you need nothing else to travel in and out of the US. You would only need a re-entry permit if you planned to stay out of the US for more than one year.
