yes you do. If you are coming from another country and obtain US citizenship then you can have dual citizenship. America doesn't recognize dual citizenship the make you choose.
No. You can hold dual or even triple citizenship. Probably more, but why would you want to?
Citizenship just extends certain rights to you within the borders of the country you're a citizen of. But it doesn't require you to relinquish citizenship to any other nation.

In this case, no because Canada allows dual citizenship. You only lose US Citizenship if you renounce it (or commit fraud in naturalization etc.) However, many countries, like Germany, make you renounce other citizenship to naturalize.
