Yes they do. They are little human "foreigners". So they do need a passport and a visa...depending on which country they come from; some countries don't need visas to come to US.
Generally, yes. Where adults don't need them, then, no.

(Little babies who 'overstay' are a lot more expensive than the adults, to be honest. It is in the cost of education and the degradation of our schools.)

Note that the babies don't come by themselves. There is an adult making a conscious decision behind that baby.
