there is some language that is required. I'm not sure what the wording is exactly, but you have to say who you are at the beginning and then at the end you have to say that the statement is a full and complete statement of truth to the best of your knowledge or something.
I think the real question here is, "Should I get a lawyer when I need to draft a legal document like an affidavit?"
To which I would respond, YES, YES you do need a lawyer!
Jessica's pretty close. A notarized statement is just swearing that that's your signature. It can still be full of lies. An affidavit usually says something like, " I (your name), being duly sworn, deposes and says.....(blah, blah, blah). I hereby certify that the statements herein are true and correct to the best of my knowledge. Signed, etc. The notary who swore you in then notarizes the document.
If you're going to provide an affidavit for something with real consequences, check with a lawyer just to be on the safe side.
