Yes, she can get it done earlier (some people recommend it), but you need to be aware of two things:
- The doctor must be approved by the embassy (contact them to get the list).
- Medical examination has a certain period of validity (may depend on the embassy -- for USCIS inside U.S. it is 1 year). Getting the K-1 may take about a year, depending on the country / your luck.

The medical must be done by a physician approved by the Embassy/Consulate in order for it to be valid, and must be done within a certain time frame (no older than 90 days I believe). What your fiance can do before the approval, is go to her doctor and make sure she has all her immunizations up to date (and get a record of them), especially if she can get the shots for free or cheaply. If the immunizations aren't up to date at the time of the immigration medical, she would have to get them then, which can be very costly.
