What do i need to include when making the authorization letter for my daughter to travel out of the country?
Answer:
I'm assuming that you are not accompanying your daughter.
If this is true, your letter should include bother her and your name, date of birth, address, phone, social security number, etc.
It should also include the name of the person you are authorizing to "transport" her out of the country.
Copies of ID (Passport/Driver's License) can be attached for reference.
I'd have it notarized and, just to minimize the risk of serious problems, accompany her to the airport.
So:
"This letter is presented as authorization for (whoever is chaperoning) to transport/accompany our daughter (name, DOB) on her trip to (place).
(Name) will be departing the US on (airline, flight) on (date) with (chaperon) bound for (destination) and will be returning on (date) from (destination).
Attached are copies of our (ID) for your reference.
Thank you,
(signed and notarized)"
Or something like that.
Is this what you were looking for?
When we took my gf son with us on vacation her letter included the authority to render medical aid, intervene if involved in any type of incident (like some countries ticket for jaywalking), permission for him to be with us and for us to act in the capacity of his surrogate parents. She also gave us a copy of his birth certificate (you are probably going to need a passport for him now) and a list of his allergies.
