What are the documents require for applying a U.S. transit visa (C-1 visa)?

I am going to Cayman Island for employment reason. I have a Cayman work permit already. My flight will have a stopover at Miami. I am sure that I need a U.S. transit visa (C-1 visa) for transiting at Miami.

However, I can’t seem to find the information regarding the requirement documents for applying the U.S. transit visa. Anybody can help with details?
