You won't have any problems. Bring the child's birth certificate or SS card just to be sure, but don't worry about it.
1st have him write an permission slip stating you have his permission to take this child out of state- 2nd have the child's birth certificate with you. You should be set to go as I used to take my son out of the country and had just his birth cert with me and never had no problems- yes we had different last names and no I wasn't the custodial parent either!
I would not expect you to have problems within the USA, but I would also recommend that you carry a copy of the child's birth certificate with your name on it, especially if you and the child differ in appearance because of the father's contribution to appearance. Some people will be alert for abducted children and some people motivated by prejudice will look for any excuse.
