Yes it is very hard for people of certain countries depending on type of immigration as immigration is limited by country.

Family based is tough for India,China,Mexico,Philliippin...
Employment based is tough for India,China and all countries if the applicant doesnt have advances degrees or lot of experience
Diversity lottery is closed for countries who have sent more than 50,000 people in thew past 5 years.
Make yourself credible and you get it

nope....its not hard, it will only take about 4 to 6 months. My brother is getting one for his wife. It isn't hard, just a little bit of paper work thats all.

Yes it is hard depending how are you going to get it. If you have a bonafide marriage with US citizen, this is the easiest way. But remember the word bonafide, the immigration officer can easily identified a fake marriage.

The other solution is through employment, family member, etc. This option could take from 2-15 years.
