I did one for my son over 30 years ago, so sorry I have forgotten what to do. If you go to one of those legal do-it-yourself form shops in many strip malls,they will be able to help you. If not, maybe someone at the local courthouse can help. good luck.
Like Marilyn, I don't remember. I changed my name 29 years ago when I got married. I do remember it was an easy back form in 1978.

I could never figure out why anyone would change their name?? its always puzzled me.
I mean i guess if you were in the witness relocation program i could understand or if your name was Harry Peters or Richard Face.
