If she uses her maiden name and that's what all her documentation states - that's the name that should be on her passport. She will need her birth certificate (CERTIFIED birth certificate) and she has to apply in person if she's never had a passport before.

She will have to use the name that is in her birth certificate or citizen certificate...If she wants her married name..she needs to start by going to the Social Security office with her marriage certificate and change her social, then the drivers licence and then she can go get her passport with her married name
