You can go to the post office and pay a fee to them. They will take your photo and get you a passport. As for the visa, I recommend you meet with an advisor at your college and ask them; they have a department that handles that.

You can get a passport with the proper identification at the United States Post Office. You need to call for location that they do that at, also they will tell you what you need. Costs money, do not know how much.

If you want to leave in December when you fill out your application tell them you want to travel in September. That way you might get it in time for your December travels. Fair warning, don't give them a December date.
Usually you get the forms at the Post Office and can pay and have it done there.
I believe you only can get one visa though. You'll have to chose between a student or a working visa. If you are going for less than six months you won't need a working visa to study.
