What is the blue form in US customs?

When my wife and I came back from Canada the US Customs agent scanned her resident card and then asked another agent for a blue form. He then made what looked like a printout of her resident card, stapled the blue form on the front and then turned the papers face-down so we could not see them. Any idea what the blue form is?
