go to www.ice.gov should answer all your questions
If it is a Machine Readable Passport i.e. has a barcode on the photo page, it should be ok. On renewal you'd need to get a biometric type passport. If your local US Consular office grants you a visa in your current passport then you'll know if you can get into the US
You didn't say what country issued your passport. In some countries, you don't need a visa to visit the US. . In many other countries you have to apply for a visa before beginning your trip to the US. Check with the US embassy or nearest US consulate in your country for details.
It will depend on your home country's relationship with the US State Dept. With most countries, your valid passport will suffice for entrance into the US, but check with the State Dept, US Consulate, and/or your home country's state dept to be on the safe side. You do need a valid passport to enter the US when flying in from any international location, even Mexico, the Carribbean, & Canada.
